The prevalence of Bone Marrow Aplasia Following Chemotherapy in Cancer Patients

dc.descriptionL’aplésie médullaire induite par la chimiothérapie constitue l’une des complications les plus graves en oncologie, avec un impact majeur sur la survie et la qualité de vie des patients. Cette étude rétrospective, menée sur une période de quinze mois dans deux centres de cancérologie, a permis d’analyser une large cohorte de patients afin de déterminer l’incidence, les issues cliniques et les profils de survie. Les résultats montrent une hétérogénéité marquée des évolutions : 40,6 % des patients ont présenté une récupération complète, 18,8 % une récupération partielle, tandis que 40,6 % sont décédés des suites de complications liées à l’aplasie. Aucun cas d’aplasie persistante n’a été observé, ce qui souligne la capacité de régénération médullaire chez les survivants. Ces données mettent en évidence à la fois la résilience du tissu hématopoïétique et la vulnérabilité des patients exposés aux cytopénies sévères. Au-delà de l’apport épidémiologique, cette recherche insiste sur la nécessité du dépistage précoce, d’une prise en charge multidisciplinaire et d’un renforcement des moyens de soutien thérapeutique pour réduire la mortalité. Elle constitue également le socle du développement de l’application APLA Risk, un outil numérique innovant destiné à aider les cliniciens à identifier les patients à haut risque d’aplasie médullaire. En traduisant les preuves cliniques en solutions pratiques, APLA Risk illustre la dimension humaine de ce travail : anticiper les complications, améliorer les décisions thérapeutiques et offrir aux patients de meilleures perspectives de survie.
dc.description.abstractChemotherapy-induced bone marrow suppression represents a significant clinical challenge in cancer treatment, with multiple agents including doxorubicin, carboplatin, cisplatin, lenalidomide, thalidomide, and vincristine identified as causative factors. The temporal pattern of myelotoxicity follows a predictable course, with alkylating agents causing acute myelosuppression. The condition presents as three primary cytopenias—neutropenia (neutrophil count <1500 cells/mm³), thrombocytopenia (platelet count <10,000/µL), and anemia (hemoglobin <10 g/dL)—with associated clinical manifestations including infections bleeding complications, and fatigue. Risk factors span patient-related elements (age, comorbidities, genetic factors), disease-related factors (cancer type and stage), and treatment related variables (cumulative dose, drug combinations, concurrent medications), with the highest risk of severe neutropenia occurring during the first chemotherapy cycle. Management strategies encompass supportive care with transfusions and myeloid growth factors, chemotherapy dose modifications or delays, and specific interventions including erythropoietin-stimulating agents and thrombopoietin. The clinical impact extends beyond hematologic parameters, as neutropenia and thrombocytopenia limit chemotherapy dosing and frequency, creating treatment dilemmas between maintaining cancer control and managing potentially life-threatening cytopenias.
