Population aging represents a major challenge for healthcare systems, particularly in digestive surgery, where elderly patients exhibit increased vulnerability due to comorbidities and age-related physiological changes. This study aimed to evaluate postoperative morbidity and mortality in elderly patients undergoing digestive surgery at the Colonel LOTFI Mixed Hospital in Laghouat, over a three-year period (March 2022 to March 2025). It is a mixed epidemiological study including 178 patients aged 65 years and older. The results showed a postoperative morbidity rate of 18.53%, with a predominance of Grade I complications (77%), according to the Clavien-Dindo classification. The observed mortality rate was 1.12%. The most significant risk factors were advanced age (> 85 years), multiple comorbidities, and a high ASA score. This study highlights the importance of rigorous preoperative evaluation, individualized management, and adapted postoperative rehabilitation in order to reduce complications and improve outcomes in this vulnerable population.
